Key Responsibilities Perform initial and ongoing assessments to develop and update personalized care plans Deliver skilled nursing services and specialized procedures per physician orders Complete timely, accurate documentation including OASIS assessments Educate patients and families on safety, medications, and health conditions Communicate with physicians regarding changes in patient condition Supervise and support LPNs and HHAs, including conducting supervisory visits Coordinate care with the interdisciplinary team and participate in team conferences Ensure compliance with all clinical, regulatory, and documentation standards Requirements What We’re Looking For Active RN license in the state of New Mexico At least 1 year of clinical nursing experience (home health preferred) Proficiency in OASIS documentation and home health regulations Strong clinical judgment and attention to detail Ability to work independently with reliable transportation Current CPR certification Compassionate communicator and collaborative team player Valid driver’s license and auto insurance Why Join Us At Corus Home Health, part of the CareM family, our mission is to bring out the courage in others.
