This role involves planning, organizing, and coordinating home care services, with a strong emphasis on community health and patient education. Key Responsibilities Develop and initiate individualized care plans in collaboration with patients and caregivers Deliver physician-ordered nursing services and implement preventive and rehabilitative procedures Educate patients and caregivers to promote health and independence Identify and implement discharge planning needs Coordinate necessary equipment and services Supervise LPNs, LVNs, and HHAs Qualifications Active California RN license Minimum 1 year of direct patient care experience (home health preferred) Current CPR certification Valid driver’s license and insured, reliable vehicle About Us Seaport Scripps Home Health is part of the Pennant Group, a nationwide network with over 300 locations.
