You will never be asked to drive to San Francisco or the East Bay. Support, Not Stress: Our back office is staffed by licensed clinicians who know exactly what you are going through. Need to call a doctor? Running late? We handle the logistics so you can focus on the patient. Quality Over Quantity: We have low production goals because we believe excellent care takes time. You won't be rushing from door to door. Financial Package We offer the stability of a salary with the upside of a performance bonus. Annual Salary: Starts at $135,000 per year (Increases based on experience & Case Management skills) Performance Bonus: Earn an additional up to 25% of your annual wage based on quality-of-care metrics. Overtime Eligible: Unlike many agencies, our clinicians are NOT Exempt. If you work late or take an extra shift, you get paid for it. Sign-On Bonus: $10,000 available for qualified candidates. Responsibilities Provide skilled nursing care in accordance with the patient's individual Plan of Care. Collaborate with the Interdisciplinary Team (PT, OT, ST, MSW) to ensure holistic outcomes. Supervise Licensed Vocational Nurses (LVNs) when assigned. Evaluate patient progress and report changes to the Case Manager or Director of Nursing. Complete documentation in a timely manner using our AI-powered charting software. Requirements Current California RN License. 1+ year of nursing experience (Home Health preferred). Valid Driver's License and reliable transportation. Compassionate, self-driven, and ready to work independently.

Responsibilities

The Registered Nurse will provide skilled nursing care according to the patient's Plan of Care and collaborate with the Interdisciplinary Team to achieve holistic outcomes. Responsibilities also include supervising LVNs and evaluating patient progress, reporting changes to the Case Manager or Director of Nursing.
