Description
Description About the Role At Touchstone Hospice, you join a team shaped by five generations of care — a place where dignity, clarity, and compassion are not only expected, but lived. As a Hospice Registered Nurse, you help bring out the courage in others by guiding patients and families through some of life’s most sacred moments. Drawing from the core purpose outlined in the job description — delivering patient-centered hospice care that preserves comfort and dignity — this role gives you meaningful clinical autonomy, steady interdisciplinary support, and the chance to make a lasting impact every single day. Key Responsibilities Complete initial and ongoing assessments and develop individualized hospice plans of care in accordance with physician orders and regulatory timelines. Provide skilled nursing services including pain and symptom management, crisis intervention, and urgent patient/caregiver support. Document clinical care accurately and timely to ensure compliance with Medicare/Medicaid Hospice Conditions of Participation. Coordinate closely with the interdisciplinary group (IDG), participate in care conferences, and ensure patients and families remain informed, supported, and empowered. Supervise and support LPNs and HHAs through required supervisory visits and delegated task accountability. Provide education to patients and families on the hospice philosophy, end-of-life care, symptom management, safety, and patient rights. Requirements What We’re Looking For Current RN license (unencumbered) in the state of practice; CPR certification required. Two years of nursing experience preferred; hospice experience strongly preferred due to role autonomy and on-call expectations. Strong clinical judgment with the ability to provide compassionate end-of-life care across homes and facilities. Emotionally steady, empathetic, patient-centered, and able to remain calm under pressure. Excellent communication, follow-through, and the ability to support families in crisis. Adaptability, integrity, and comfort working independently while staying connected to a highly supportive team. Valid driver’s license, auto insurance, and reliable transportation for home visits. Why Join Us This work is meaningful — and so are the people you’ll do it with. Touchstone Hospice is grounded in CareM’s mission to bring out the courage in others, and you’ll feel that mission in the way we support our patients, our families, and each other.
