Position Description

Position Purpose

  • OSAN Registered Nurses provide high-quality nursing care and support to clients in their homes and the community. This role ensures the delivery of person-centred care that meets individual needs, promotes independence, and supports overall well-being. The position also includes education and collaboration with clients, their families, and other healthcare professionals to achieve optimal health outcomes.

Main Tasks

  • Health Monitoring and Management:
  • Blood pressure and blood sugar monitoring.
  • Pain and medication management.
  • Wound care, including dressing changes and infection prevention.
  • Respiratory care (e.g., nebulisers, asthma education).
  • Condition Management and Education:
  • Providing education on client-specific conditions such as diabetes and lifestyle modifications.
  • Dietary management and hydration monitoring.
  • Specialised Care Services:
  • Palliative care, dementia support, and mental health support.
  • Continence management, including catheter care, stoma care, and toileting schedules.
  • Falls risk assessment and prevention strategies.
  • Coordination and Collaboration:
  • Liaising with allied health professionals for holistic client care.
  • Supporting carer education to enhance care delivery.
  • Disability and Specialist Care:
  • Behaviour and therapeutic support.
  • Supporting the use of assistive technologies and home modifications.
  • Managing Specialist Disability Accommodation (SDA).

Other Duties

  • Attend team meetings, training sessions, and professional development activities.
  • Maintain accurate and timely documentation of client care and progress.
  • Adhere to organisational policies, procedures, and regulatory compliance requirements

Person Specification

Qualifications

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
  • Valid First Aid and CPR certification.
  • Postgraduate qualifications in areas such as community health, mental health, or aged care (desirable).

Experience

  • Minimum 2 years’ experience in community or home nursing settings.
  • Experience in managing chronic conditions, palliative care, and dementia support.
  • Familiarity with disability care and NDIS guidelines.

Skills & Competencies

  • Clinical skills in medication, wound, and continence management.
  • Strong documentation and reporting abilities.
  • Competence in using assistive technology and implementing care plans.
