This unique dual-role position blends hands-on clinical nursing with patient education and chronic disease management, allowing you to build meaningful relationships while improving health outcomes for underserved populations. What You’ll Do Clinical Nursing & Patient Care Provide direct nursing care across the lifespan in a primary care setting Perform clinical tasks including vital signs, medication administration, specimen collection, and CLIA-waived testing Assist providers during exams and procedures Maintain accurate documentation in the EHR Follow all safety, infection control, and HIPAA standards Patient Education & Chronic Disease Management Deliver individualized education on chronic conditions such as diabetes, hypertension, asthma, and preventive care Use culturally and linguistically appropriate teaching methods Support patient self-management through motivational interviewing and goal setting Care Coordination & Collaboration Coordinate care with providers and interdisciplinary team members Identify and help address barriers to care (transportation, medication access, health literacy) Refer patients to community resources and support services Leadership & Outreach Provide guidance and support to LPNs, MAs, and clinic staff as needed Participate in team huddles, quality improvement initiatives, and case reviews Assist with community outreach events, wellness fairs, and health education programs Why Join Complete Health?Mission-driven work serving vulnerable and underserved communitiesTeam-based, supportive clinic environmentOpportunity to blend clinical care with patient education and advocacyMeaningful work that directly improves patient lives Complete Health is an equal opportunity employer. Requirements Required Current, unrestricted RN license in South Dakota BLS/CPR certification Minimum 2 years of nursing experience Preferred Experience in community health, primary care, public health, or FQHC settings Background in patient education or chronic disease management Skills That Set You Apart Strong communication and interpersonal skills Cultural humility and commitment to health equity Comfort working with diverse, underserved populations Proficiency with EHR systems and clinical documentation
