As part of our Client & Staff Services Team, you will join a growing group of professionals who provide high-quality nursing care directly in clients’ homes or community environments. You will promote and support client safety, health, and independence while fostering strong relationships with clients, families, caregivers, and other healthcare partners.

QUALIFICATIONS

  • Education: Bachelor of Science in Nursing (BScN) from a recognized university, college, Indigenous institute, or college-university partnership.
  • Registration: Current registration and good standing with the College of Nurses of Ontario (CNO).
  • Training: Demonstrated experience in community nursing, chronic disease management, wound care, medication administration, and health education.
  • Experience: Minimum 1–2 years of recent nursing experience preferably in home care, community health, or hospital settings.
  • Certifications: Current CPR and First Aid certification (Advanced CPR preferred).
  • Screening: Current Vulnerable Sector Check.
  • Physical Requirements: Ability to perform nursing tasks, including lifting, positioning, and mobility assistance.
  • Transportation: Valid driver’s license and reliable transportation to visit clients in the community.

Responsibilities
  • Conduct comprehensive nursing assessments of clients in their homes or community settings.
  • Develop, implement, and evaluate individualized care plans based on client needs and preferences.
  • Administer medications, treatments, and wound care as prescribed by physicians or nurse practitioners.
  • Monitor and document changes in client health status, responding promptly to concerns.
  • Educate clients and caregivers on managing health conditions, medications, and wellness practices.
  • Provide specialized care to pediatric clients, including developmental assessments, family education, and condition-specific support.
  • Collaborate with interdisciplinary teams to coordinate care and ensure continuity.
  • Provide crisis intervention and emergency response as needed in the home/community setting.
  • Maintain accurate, timely documentation in accordance with professional standards and CareOnDemand policies.
  • Supervise and support RPNs, PSWs, and other care team members as applicable.
  • Promote a safe, respectful, and compassionate care environment in every interaction.
