JOB SUMMARY:

Organize and administers individualized age specific professional care to patients within an assigned unit of the hospital in support of medical care. Supports philosophy, objectives and the mission and vision of Saint Barnabas Medical Center and the Department of Nursing. Practices in accordance with the State Nurse Practice Act, American Nurses Association Standards of Practice, Code of Ethics for Nurses, Department of Health Regulations, JCAHO requirements and other regulatory agencies. Participates in patient teaching and discharge planning. Participates in unit Performance Improvement evaluation activities.

EDUCATION:

  • Must be a graduate from an accredited school of nursing.
  • Baccalaureate Degree in Nursing.

REQUIREMENTS:

  • Current New Jersey RN licensure required.
  • BLS certification required. (American Heart Association)
  • Must be I.V. certified from an approved facility or completion of requirements for I.V. certification within 90 days of hire to unit.
  • Completion of all Orientation requirements of Saint Barnabas Medical Center.
  • Successful completion of all Orientation Programs.

Responsibilities
  • Assist patient and family members in coping by encouraging oral expression of feelings associated with fears and losses imposed by disease andtreatment and makes referrals to Social Services as necessary,
  • Assists patient in the use of ambulation techniques and assistive devices such as: commode chair, walker, hip chair, and crutches, etc, and positioningof patient,
  • Assumes Charge Nurse functions as delegated by the PCC and/or Asst, Dir, Of Clinical Svcs, including assessing staffing needs, organizing anddelegating work assignments based on patient acuity and individual staff skill levels,
  • Complies with Medication Reconciliation policy,
  • Complies with unit and departmental scheduling guidelines,
  • Delegates and prioritizes work assignments according to the individual associate’s competence and patient care needs,
  • Demonstrates proper procedure care for and maintenance of central lines; flushing, blood draws and dressing changes,
  • Demonstrates the ability to recognize hematological and oncological emergencies and takes appropriate action when one occurs,
  • Demonstrates understanding of hospital policy and procedure regarding restraints,
  • Demonstrates understanding of/states blood administrative hospital procedure,
  • Complies with blood and blood product administration policy,
  • Develops, implements and evaluates a comprehensive plan of care for daily management of hematology/oncology patients; sickle cells crisis,chemotherapy, fever, neutropenia,
  • Evaluates effectiveness, interventions and revises plan of care accordingly,
  • Implements the policies and procedures of the hospital and Nursing Department,
  • Initiates, maintains and monitors care of a medication extravasation using established policy and procedure,
  • Initiates, maintains and monitors IV chemotherapy and implanted access devices, observing for side effects and bio hazardous exposure,
  • Initiates/develops multidisciplinary care plans and discharge plans in accordance with department standards and using appropriate tools,
  • Integrates psychosocial, physical, nutritional and rehabilitation needs of the patient in patient care and discharge planning,
  • Performs and documents accurate neurological assessment as indicated by patient condition,
  • Performs assessment and reassessment of patients according to applicable department standards and identified tools,
  • Performs interventions in accordance with current policy and procedure, safety guidelines and infection control principles,
  • Provides appropriate oncology/chemotherapy related education to patients and families as per established standards of care, policies, and procedures,
  • Provides patient/family education according to the plan of care and documents same,
  • Sets goals and formulates appropriate multidisciplinary interventions to meet these goals,
  • States proper mechanism for activating cardiac respiratory arrest, Demonstrates understanding of code cart check and procedure for replacing itemson code cart,
  • Transmits pertinent patient information to appropriate individual i, e, MD, supervisor, discharge planner, etc,
  • Uses an understanding of the principles of acute, chronic and oncology pain management to assess each patient s individual needs and provideappropriate and specific treatment and care,
  • Uses an understanding of the principles of growth and development to assess each patients age specific needs and provide age specific treatment andcare,
  • Utilizes the nursing process with a multidisciplinary approach in the delivery of patient care,
