The RN will ensure quality care, monitor outcomes, and work closely with caregivers, families, and healthcare professionals. Key Responsibilities: Conduct comprehensive in-home assessments and reassessments Create and revise individualized care plans based on clinical findings Provide medication administration and skilled nursing interventions Monitor and document changes in client health and status Communicate regularly with clients, caregivers, and care teams Initiate referrals to appropriate healthcare or community resources Maintain timely, accurate documentation in accordance with regulations Participate and facilitate caregiver in-service programs. Qualifications: Current Maryland Registered Nurse (RN) license or mutli-state licensure. CPR/First Aid Certification Experience in Home Health, Hospice, and/or Geriatric Nursing preferred Strong assessment and clinical decision-making skills Ability to work evening and weekend shifts, as needed Reliable transportation for home visits.
