RPN / RN Nurse Care Manager (Care Designer)
Location: Mississauga, Ontario
89 Queensway W, Mississauga, ON
(next door to Mississauga Hospital)
Type: Full-time
Schedule: Flexible availability, including evenings, weekends, holidays, and on-call duties. Schedule provided 2 months in advance.
Key Responsibilities

CLIENT CARE DELIVERY & EXPERIENCE

Attend all First Visits with caregivers to ensure quality care and alignment with service agreements.
Conduct supervisory visits to monitor caregiver fit and care quality.
Develop and update individualized care plans.
Educate clients on self-care and monitor risks/outcomes.
