Responsibilities

We’re seeking a clinician to work in the Transitional Care Program (TCP). This role involves both clinical work and client coordination.

We’ll also be asking you to:

  • Regularly liaise with our Care Coordinators & external allied health / medical professionals.
  • Share your clinical knowledge. Support, mentor & train our Enrolled Nurses.
  • Undertake wound care, monitor medication. Assist with chronic illness & continence.
  • Develop and maintain trust with our clients.
  • Go above & beyond to assist clients achieve desired health outcomes.
