SOUTHLAKE ACADEMIC FAMILY HEALTH TEAM (SAFHT)

The Southlake Academic Family Health Team (SAFHT) is seeking a dedicated and compassionate Registered Nurse (RN) to join our growing interprofessional team in a full-time, permanent capacity. While the primary location for this role is our Newmarket office, occasional work may be required at our Aurora, Bradford, or Mt. Albert sites.
As a valued member of our team, the RN will support the mission and vision of SAFHT by delivering comprehensive, patient centered primary care across the lifespan. This includes promoting health, preventing disease, and supporting chronic disease management within the full RN scope of practice and in collaboration with our interdisciplinary care team.

QUALIFICATIONS

  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Minimum of 1 year experience in a primary care setting (experience in an academic or teaching environment considered an asset)
  • Proven ability to work independently with strong organizational and time management skills
  • Proficient in the use of electronic medical records (EMRs) and general computer applications
  • Strong verbal and written communication skills
  • Collaborative mindset with a commitment to working in interprofessional teams and supporting learners
  • Current CPR HCP certification

Responsibilities
  • Provide direct primary care to patients of all ages

  • Support health promotion, preventative screening, and chronic disease management
  • Contribute to program development and quality improvement initiatives to enhance patient care and outcomes
  • Participate in the orientation and support of interprofessional learners and students
  • Deliver care within clinic and community settings, including home visits
  • Perform additional duties as required to support the evolving needs of the team
