About the role We are excited to offer an opportunity to join our dynamic and growing team supporting clients through the Commonwealth Home Support Programme (CHSP). This role provides a rewarding chance to make a meaningful difference by delivering person-centred care to older people living in the community. Play a key role in providing high-quality nursing care to clients referred to our entry-level aged care services. Work within a supportive and collaborative multidisciplinary team, you will undertake comprehensive clinical assessments, develop care and discharge plans, and deliver therapeutic interventions tailored to each client’s individual needs. Case management for older clients in their homes, supporting them to maintain independence, safety, and quality of life within their community. We operate Monday to Sunday, including public holidays. About you Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency (AHPRA). Minimum of 2 years’ experience in community setting. Current unrestricted Class C Driver’s Licence (or above). Demonstrated ability to work collaboratively within an interdisciplinary team, with strong communication and interpersonal skills Commitment to ongoing professional development and demonstrated computer literacy, including email and electronic patient record systems.
