ABOUT THE ROLE

As a Registered Nurse in the Geriatric Evaluation Management (GEM) and Dementia Management Unit (DMU) at Sunshine Hospital, you will play a pivotal role in delivering high-quality, person-centred care to older adults with complex needs. Working within a multidisciplinary team, you will be responsible for assessing, planning, and implementing evidence-based nursing interventions to support patients’ recovery, independence, and wellbeing. KEY RESPONSIBILITIES:

  • Deliver evidence-based, person-centred clinical care to patients in the GEM and DMU wards, including assessment, risk minimisation, care planning, and discharge planning.
  • Collaborate effectively within a multidisciplinary team to coordinate care, provide health education, and support decision-making aligned with patients’ goals.
  • Demonstrate leadership by supervising early career staff, contributing to peer feedback, and supporting preceptorship and staff development.
  • Actively participate in quality improvement initiatives, research, and clinical audits to enhance service delivery and patient outcomes.
  • Maintain accurate, timely, and legally compliant clinical documentation in accordance with Western Health standards and guidelines.
