Psychosis Recovery & Treatment Program (PRT) is seeking is seeking a full-time permanent, Registered Nurse who will work in the Transition Service. This clinic supports clients transitioning from outpatient PRT clinics to their family health care team or to external community providers using critical time intervention approach, client-centered and recovery principles. The Transition Service Registered Nurse will: work as a member of the inter-professional team, provide case management support, outreach support, perform ongoing psychiatric and physical assessments, and provide medication administration. The Transition Service Registered Nurse will also: develop, implement and evaluate inter-professional service plans for clients who have schizophrenia, primary psychotic disorder and concurrent disorders, in accordance with professional standards as set out by the College of Nurses of Ontario. You will act as a system navigator, liaising these clients with appropriate services and resources. You will work in collaborative partnerships with clients, their formal and informal supports, and the family health care teams for a successful transition of care. You will work with clients from diverse populations; participate in clinical supervision or reflective practice; contribute to program development; provide education support and supervision to students and participate in the research activities of the program. Your skills include the ability to make behavioural observations, complete measurement-base care assessments, mental status and nursing physical health exam assessments, you will demonstrate the ability to monitor clients’ response to pharmaceutical and care interventions. You will have sound knowledge of community resources, crisis intervention, recovery, trauma-informed care, addiction issues and severe and persistent mental illness. You will support and contribute to a workplace that embraces diversity, encourages teamwork and complies with all applicable legislative and regulatory requirements. This position is located at 1051 Queen Street West.
Job Requirements
The successful candidate must be a member in good standing with the College of Nurses of Ontario and possess a current B.C.L.S.(CPR). You will have two years of nursing experience. Preferred experience in psychiatric/mental health nursing. Experience within an outpatient setting with sound knowledge in crisis intervention and in rapid intake assessment is an asset. Preference will be given to candidates who possess or are currently pursuing a B.Sc. N and/or CNA certification in Psychiatric/Mental Health Nursing. Preference will also be given to those with community nursing experience, and a comprehensive understanding of Recovery principles and their application to client service. You will possess excellent communication and interpersonal skills to enable you to develop the Nurse-Client relationship and work within a collaborative and diverse inter-professional team. Effective leadership, decision-making, organizational, and problem solving skills are required. Knowledge of emergency psychiatry and crisis intervention principles and theory are required. The ability to work with clients of diverse cultural and social backgrounds is necessary. Computer literacy is required. Bilingualism (French/English) or proficiency in a second language would be an asset.
