Key responsibilities include:

• Clinical assessment and intervention across a range of programs;
• Implementation and review of care plans in line with consumer directed care;
• Education and training, including clients, staff and multidisciplinary teams;
• Responding to new referrals and changing needs of clients;
• Collaboration with therapy service providers to coordinate holistic care.

Now... to ensure our consumers can get to work with you, it is essential you hold: 

• A tertiary qualification in Nursing (or similar); 
• AHPRA registration; 
• Experience within an aged care or a similar setting;
• NDIS Worker Screening Check (or willingness to obtain) and 
• Full working rights in Australia.
