Description

Madison Village, a 78-bed long-term care home in Stoney Creek, Ontario is an exciting long term care home that is dedicated to enriching the lives of our residents in partnership with UniversalCare Health Services, our family members, valued employees and community partners.
Our team is currently hiring for 1 permanent part-time Registered Physiotherapists (15 hours per week).
Qualification, Duties and Responsibilities:
Successful completion of Degree in Physiotherapy
Minimum one to two (1-2) years long term care experience
Current registration with the College of Physiotherapists of Ontario in good standing
Membership in the Canadian Physiotherapy Association preferred
Excellent interpersonal skills
Excellent verbal and written communication skills
Excellent organizational and time management skills
Excellent problem solving skills
Must be able to perform the physical demands of the job
Ability to work independently and in a group setting
Clinical Care: Assessment and re-assessment;
Treatment - individual and group program(s);

Other duties:

  • Providing in-services and various educational seminars including nursing rehab/restorative programs to staff of long term care
  • Participate in multidisciplinary care conferences;
  • Documentation as required by the MOHLTC and long term care home including MDS coding and care planning;
  • Providing leadership and attending various committees including restraint reduction, fall prevention
  • Ongoing communication with long term care home management, nursing and other team members;
  • Supervision of PTA’s;
  • Excellent customer service skills relating to the needs and challenges of residents and their families;
  • Awareness to maintain confidentiality pertaining to the long term care home and resident information.
  • Update and liaise with nursing, PSW’s and the team regarding defective equipment, wheelchairs, and mobility aids
  • ADP authorizer
