Description
The Registered Practical Nurse (RPN) is a key member of the interdisciplinary ALS Clinic team, providing coordinated, patient- and family-centered care to individuals living with Amyotrophic Lateral Sclerosis. Reporting to the Clinical Manager, Urgent & Ambulatory Care, the RPN works collaboratively with physicians, nurses, and allied health professionals to support team-based care delivery. The RPN supports the clinical flow of ALS clinic visits, including patient rooming, collection of vital signs and weights, medication reconciliation, and completion of standardized tools such as the ALS Functional Rating Scale (ALSFRS). This role supports timely assessments, continuity of care, and efficient clinic operations. The RPN also supports virtual care by completing pre- and post-visit activities for virtual and in-clinic appointments, including coordination, documentation, and patient follow-up as appropriate. The core focus of the role is collaboration across the interdisciplinary team to support coordinated care planning, effective communication, and seamless care transitions for patients and caregivers. The RPN practices in accordance with College of Nurses of Ontario standards and organizational policies and contributes to quality improvement, learner support, and ongoing program development within the ALS Clinic. Current Certificate of Registration from the College of Nurses of Ontario as a Registered Practical Nurse Current Basic Cardiac Life Support (BCLS) certification Recent experience, within the last 3 years, in a tertiary care outpatient setting Demonstrated knowledge and understanding of the approach to ALS assessment, care planning, intervention and follow-up Previous experience with clinic coordination or patient navigation preferred Previous experience with managing clinic flow, obtaining vital signs and patient data, communicating results to the interdisciplinary team, and coordinating follow-up Excellent teamwork, communication, problem-solving, and organizational skills Demonstrated knowledge, skill and ability in health teaching with patients and their families individually and in a group setting Demonstrated ability to communicate with patients and their families under stressful conditions Knowledge, skill and ability to provide health teaching for patients and their families, specific to the needs of the clinic population Demonstrated knowledge of motivational interviewing Demonstrated skills in psychosocial assessment Ability to adapt to rapid change in patient condition workload status Demonstrated leadership abilities and the ability to organize and coordinate patient and family-centered care with other members of the healthcare team Demonstrated ability to communicate (written and verbal) effectively with patients, families, and other members of the healthcare team. Demonstrated skill and ability in management and triage of patient referrals Demonstrated ability to function within an interprofessional team, including nursing collaboration with RNs and other members of the healthcare team. Demonstrated ability to assess concurrent demands, set priorities, organize work, and maintain accuracy and concentration in periods of high activity in response to patient and team needs. Demonstrates ongoing personal professional education and development Demonstrated computer proficiency and experience with EPIC & MS Office. Experience with EPIC workflows and EMR best practices preferred
