We are looking for a detail-oriented Registered Practical Nurse (RPN) to join our long-term care team. The ideal candidate will have strong observational skills, ensuring accurate documentation and thorough monitoring of residents’ health conditions. Key Responsibilities:

  • Resident Care:
  • Provide direct care to residents, including assistance with daily activities such as bathing, dressing, and grooming.
  • Administer prescribed medications and treatments according to established protocols.
  • Monitor vital signs, record health data, and report any changes in condition to the charge nurse or physician.
  • Perform wound care and other clinical tasks as required, ensuring proper documentation.
  • Assist in the development, implementation, and evaluation of resident care plans in collaboration with other healthcare providers.
  • Health Monitoring & Documentation:
  • Conduct regular assessments of residents’ physical and mental health conditions.
  • Record accurate and timely documentation in residents’ medical records, adhering to confidentiality and legal standards.
  • Monitor and report any signs of infection, deterioration, or discomfort to the interdisciplinary healthcare team.
  • Assist in the management of chronic conditions such as diabetes, heart disease, and mobility issues.
  • Team Collaboration:
  • Work as part of a multidisciplinary team, including RNs, physicians, personal support workers (PSWs), and other healthcare professionals.
  • Communicate effectively with residents, their families, and the care team to ensure the highest standard of care.
  • Participate in case conferences, staff meetings, and ongoing education and training.
  • Emergency and Crisis Response:
  • Respond to medical emergencies, providing immediate care and escalating to higher medical staff if necessary.
  • Administer first aid or CPR when required and assist in stabilizing residents until advanced care arrives.
  • Resident Advocacy and Support:
  • Advocate for residents’ rights, ensuring their dignity, autonomy, and well-being are respected.
  • Provide emotional support to residents and their families, addressing concerns and providing information about care procedures.
  • Help facilitate recreational and social activities to enhance residents’ quality of life.
  • Health and Safety Compliance:
  • Adhere to infection control protocols and safety regulations to prevent the spread of illnesses.
  • Maintain a clean and safe working environment, following all health and safety standards.
  • Participate in training related to patient safety, infection prevention, and other relevant topics.

Qualifications:

  • Education:
  • Completion of a Practical Nursing Program from an accredited institution.
  • Licensure:
  • Current registration as a Registered Practical Nurse (RPN) with the College of Nurses of Ontario (CNO).
  • Experience:
  • Previous experience in long-term care, geriatric care, or rehabilitation is an asset.
  • Knowledge of common medical conditions affecting elderly populations, including dementia, mobility issues, and chronic illnesses.
  • Skills:
  • Excellent interpersonal and communication skills.
  • Strong organizational and time-management abilities.
  • Ability to work independently and as part of a team.
  • Compassionate, empathetic, and patient-centered approach to care.

Physical Demands:

  • Ability to assist with the lifting and moving of residents.
  • Ability to stand and walk for extended periods.

Work Environment:

  • The role will require working in a long-term care setting, with a focus on direct patient interaction in a residential home environment.
  • Regular shift work, including evenings, nights, and weekends, as required.
