POSITION SUMMARY:

We are currently seeking a dedicated Registered Practical Nurse (RPN) to join our interdisciplinary care team. The RPN plays a vital role in the health and well-being of residents, providing direct nursing care and ensuring adherence to individualized care plans. This is an excellent opportunity for someone passionate about geriatrics and long-term care.

QUALIFICATIONS:

  • Current registration in good standing with the College of Nurses of Ontario (CNO)
  • Practical Nursing Diploma from an accredited institution
  • Experience in long-term care, geriatric nursing, or a related field preferred
  • Current CPR and First Aid certification
  • Strong assessment, communication, and interpersonal skills
  • Ability to work effectively in a team-based environment
  • Knowledge of electronic charting systems (e.g., PointClickCare) is an asset
  • Availability to work flexible shifts, including evenings, nights, weekends, and holidays

Responsibilities
  • Provide safe, effective, and ethical nursing care to residents in accordance with the College of Nurses of Ontario (CNO) standards.
  • Administer medications and treatments as prescribed and document appropriately.
  • Collaborate with Registered Nurses (RNs), Personal Support Workers (PSWs), physicians, and allied health professionals.
  • Monitor and assess residents’ health status, reporting any changes in condition.
  • Develop, implement, and evaluate resident-centered care plans.
  • Promote a safe and respectful environment for residents, families, and staff.
  • Maintain accurate records and documentation in accordance with professional and regulatory standards.
  • Support infection prevention and control practices within the home.
  • Participate in shift change reports, team meetings, and continuing education initiatives.
