In collaboration with an RN provides nursing assessment, care plan development, goal oriented treatment interventions, patient/family education and support to address medical cognitive, emotional and self-care needs within scope of practice. Contributes to the development of a realistic discharge plan. The ability to perform as a coach/mentor/consultant and to use sound critical thinking skills is key to achieving patient outcomes. Participates in program planning, continuous quality improvement and evaluation. Key Qualifications: A current annual registration card with the College of Nurses of Ontario. Recent (2 years) Medication and Aseptic Technique Certifications. Current Physical Assessment course preferred. Recent (1-3 years) experience with advanced clinical skills in management of Stroke patients throughout the continuum of care from acute through rehabilitation. Neurological/neurosurgical, cardiac and medical experience would be an asset. Current BCLS and evidence of continued education required. Combined with sound patient care nursing skills must have effective interpersonal and organizational skills. Reliable, flexible, creative, self-directed professional able to work effectively and collaboratively with the Stroke Team. Recent (1-3 years) experience working with a high performance interdisciplinary team required. Demonstrated effective interpersonal, organizational and communication skills are required. Positive attendance and work record essential. Must meet the physical demands of the position; performing substantial walking, standing, lifting, twisting and bending as required for patient care and other responsibilities. Stroke experience an asset. Positive work and attendance record is essential.
