About McCall Centre: McCall Centre is a 120-bed chronic care facility situated on the Queensway Health Centre campus. Our specialized inter-professional teams will work with families to support patients’ plan of care and goals with compassion and excellence. Position Summary Responsible and accountable for decisions and actions of the provision and management of nursing care for patients; Provides nursing assessment, contributes to the care plan development, provides goal oriented treatment/interventions, patient and family education and support to address medical, cognitive, emotional and self-care needs; Contributes to the development of a realistic discharge plan Acts as a coach/mentor/consultant and uses sound critical thinking skills which are key to achieving patient outcomes and facilitating team functions; Participates in program planning, continuous quality improvement and evaluation.
