RESPONSIBILITIES Perform comprehensive assessments and deliver personalized nursing care during house visits. Assist with vaccination clinics and injections of prescribed medications. Perform point of care diagnostic testing under medical directives. Educate patients and their families about health management and medication adherence. Maintain accurate documentation of all visits and interventions. Other duties as assigned. QUALIFICATIONS Current registration in good standing as a Registered Practical Nurse (RPN) with the College of Nurses of Ontario. Demonstrated ability to work independently and collaboratively within a multidisciplinary care team. Strong communication, interpersonal, and patient engagement skills. Excellent clinical judgment, organizational skills, and attention to detail. Valid Class G driver’s license and a driving record in good standing SKILLS Strong clinical assessment and critical thinking skills to support high-quality patient care. Excellent communication and interpersonal skills to build trust with patients, families, and the care team. Ability to work independently during home visits while maintaining professionalism and sound judgment. Detail-oriented with strong documentation and record-keeping abilities. Skilled in administering injections, vaccinations, and performing point-of-care testing accurately. Compassionate and patient-centered approach to healthcare delivery. Proficient in using electronic medical records (EMR) and other digital tools for charting and reporting. Strong time management and organizational skills to manage multiple patient visits efficiently. Commitment to confidentiality, ethical standards, and compliance with nursing regulations.
