Our specialized team can support patients’ care plan goals while providing more social engagement and recreational therapies. Responsibilities: Working as a member of an Inter-professional team to provide quality care to the ALC patient population Responsible for the provision of nursing care, teaching and coaching to patients and their families Responsible and accountable for the provision of nursing care in accordance with the standards of nursing practice and THP Best Practice Protocols Key Qualifications: A current registration in good standing with the College of Nurses of Ontario required Medication Certificate and experience with administration of medication within the last year required Current BCLS certificate Previous experience in complex continuing care, long term care, rehab nursing concepts, gerontology and treatment approaches required Experience with patient centred and partnership care models Ability to think critically and effectively in order to prioritize needs High effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written Reliable and flexible individual who is willing to work collaboratively within an interdisciplinary team environment Positive attendance and work record essential To pursue this career opportunity, please visit our website: www.trilliumhealthpartners.ca Candidates are selected on the basis of their skill, ability, experience and qualifications.
