ALIGN ResponsibilitiesAs scheduling permits, the RPN will support patients as they transition from hospital to home, helping to ensure a safe and successful discharge experience. Responsibilities include, but are not limited to, conducting in-home assessments, monitoring patients' clinical status and recovery, assessing vital signs and symptom management, reviewing medications for adherence and understanding, providing disease-specific education and self-management coaching, reinforcing discharge instructions, assessing home safety and functional needs, identifying early signs of deterioration or complications, and implementing appropriate interventions within their scope of practice.

• Qualifications:Current registration in good standing with the College of Nurses of Ontario (CNO).
• Current Basic Cardiac Life Support (BCLS) certification.
• Recent medical, surgical, geriatric, or community nursing experience is preferred.
• Demonstrated ability to work independently and make sound clinical decisions.
• Strong communication, assessment, and care coordination skills.
• Excellent organizational and time-management abilities.
