The Belleville Nurse Practitioner-Led Clinic (BNPLC) is seeking a Registered Practical Nurse (RPN) to join our interdisciplinary health care team. Since 2010, the BNPLC has been a trusted leader in providing accessible, individualized, fully responsive primary health care in the Quinte Region. With more than 3500 registered patients with a diverse range of needs, we make a difference every day.

POSITION SUMMARY

The Registered Practical Nurse (RPN) assists in the delivery of primary care services, wellness education and care coordination services for clinic patients. The RPN will work collaboratively with the primary healthcare team to provide patient-centered, quality primary health care to our patients.
The RPN will be a member of the Registered Practical Nurses Association of Ontario (WeRPN) to ensure liability protection and to participate in professional responsibility and leadership.
The Registered Practical Nurse (RPN) plays a key role in supporting primary care services by serving as the first point of contact for patients, whether in person or by phone. The RPN is responsible for assessing and appropriately triaging patient needs while delivering a blend of clinical, administrative, and triage services tailored to each individual. Working in a respectful and team-oriented environment, the RPN will practice to the full extent of their scope, ensuring safe and effective care in alignment with the standards of their professional college.

EDUCATION / EXPERIENCE:

  • Diploma in Nursing from an accredited university
  • Registration as an RPN with the College of Nurses of Ontario
  • Membership with WeRPN
  • Exceptional understanding of current, evidence based, and best practice guidelines
  • Excellent communication and organizational skills
  • Experience with an Electronic Medical Record, e.g., Telus EMR
  • Demonstrated ability to work independently and in a team environment
  • Experience in program design, delivery, and evaluation an asset
  • Professional liability insurance
  • Current certification in CPR

JOB REQUIREMENTS:

  • Adhere to Occupational Health and Safety policies and procedures as per applicable legislation
  • Adhere to all policies and procedures of the Belleville NPLC
  • Current Police Vulnerable Sector Check (less than 6 months old)
  • Vaccinated against Covid-19 (minimum of two doses) or has valid medical exemption

Responsibilities

DUTIES AND RESPONSIBILITIES:

  • Provide first level triage, hear patient needs and refer to the next appropriate level as warranted;
  • Provide telephone advice and document all advice and care (per CNO standards of practice) in the Electronic Medical Record
  • Make timely decisions and prioritize relative to the type and urgency of the patient’s need;
  • Supports the triage of information from laboratory and diagnostic results and ensures That the Nurse Practitioner receives any critical results in a timely manner
  • Perform physical and holistic patient assessments, triages information, navigates the health system to ensure that patients’ needs are met;
  • Contribute to effective provision of primary care services by scheduling patientappointments;
  • Provide reception functions as needed;
  • Scan, upload and e-fax patient records as required;
  • Ensure that clinical areas, resources, materials and equipment are maintained;
  • Foster an enthusiastic team spirit through nurturing positive relationships among staff;
  • Provide health promotion and education
  • Collaborate and/or consult as appropriate, with members of an interdisciplinary team within the clinic
  • Advocate, educate, and consult as needed with the /team members.
  • Participate in committees or community groups where an identified need for advocacy or representation is required
  • Other duties as may be assigned

ADDITIONAL RESPONSIBILITIES

  • Record nursing and medical interventions according to CNO standards of documentation
  • Submit statistics as required by the BNPLC
  • Participate in training sessions as required by the BNPLC
  • Participate in regular team meetings, and other meetings as requested
  • Participate in the development, planning and evaluation of treatment, education, counselling, and health promotion/disease prevention activities of the BNPLC.
  • Assist in the maintenance of a clean, orderly work environment
