REGISTERED PRACTICAL NURSE (RPN) – TRACHEOSTOMY CARE SPECIALIST

We are seeking a Registered Practical Nurse (RPN) who is passionate about providing high-quality care to seniors and skilled in supporting clients with specialized medical needs, including tracheostomy care, in a home care setting.

SKILLS AND QUALIFICATIONS:

  • Proven experience in home care, particularly with tracheotomy patients.
  • Strong knowledge of sanitation, equipment maintenance, and troubleshooting for tracheotomy care.
  • Proficiency in pain management and medication administration.
  • Excellent communication skills to liaise with families and care teams.
  • Experience with catheter care and monitoring is an asset.

REQUIREMENTS:

  • Certifications:
  • Clear Vulnerable Sector Check.
  • Current CPR and First Aid Certification.
  • Up-to-date immunizations, including flu shot, TB test, diphtheria/tetanus, MMR, varicella/zoster, hepatitis B, and COVID-19 (fully vaccinated).
  • Transportation:
  • Valid driver’s license, car insurance, and reliable vehicle.
  • Must have reliable transportation as client needs are high risk
  • Availability:
  • Shift: 6:15 a.m. to 2:30 p.m.
  • Full-time schedule (40 hours weekly).
Responsibilities
  • Deliver patient-centered care in home settings, ensuring dignity and respect.
  • Perform comprehensive assessments and develop tailored care plans.
  • Administer medications and treatments as prescribed by healthcare providers.
  • Provide expert care for tracheostomy patients, including:
  • Routine tracheostomy site care and maintenance.
  • Suctioning as needed to ensure airway patency.
  • Monitoring the site for any signs of infection or complications.
  • Assist with tube feeding as required.
  • Support clients with personal care, daily living activities, and cognitive exercises.
  • Perform deep housekeeping tasks such as cleaning, vacuuming, sweeping, mopping, wiping down furniture, cleaning equipment, laundry, tube feed administering, cleaning and disinfecting equipment, washing dishes, cleaning kitchen, cleaning and disinfecting bathroom and garbage and recycling disposal.
  • Maintain clear documentation using electronic health record systems (e.g., Axis Care).
  • Collaborate with families and interdisciplinary teams to achieve optimal outcomes for clients.
