Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.   About Cherry Creek Primary Health Service: Barwon Health is partnered with the Department of Justice and Community Safety to provide the health care service at the Cherry Creek Youth Justice Centre. Cherry Creek is a 140 bed Youth Justice Facility to the west of Werribee in the vicinity of Cherry Creek that accommodates males between 15-17 years of age, both sentenced and on remand. The services delivered include emergency response (with support from the Barwon Health Virtual ED), primary care beds, integrated mental health, drug and alcohol services, allied health, general practice and specialist medical care as required. There is 24/7 nursing coverage on-site as well as a 7-days a week primary care clinic. Diagnostic (pathology and imaging) and pharmacy is also be available on site. The health care is integrated in the operations of the Centre and is primarily operating from a state of the art purpose built health facility. This is how we ensure Cherry Creek enhances the outcomes we achieve for the young people in our care and the community.   About the Role Provide primary health-level assessment, treatment and recovery based interventions to Young People in custody at Cherry Creek who are experiencing mental health difficulties. Provide a range of harm reduction services and education, including delivery of manualised AOD programs, to groups and individual Young People in custody at Cherry Creek. Facilitate holistic care by providing appropriate support, linkages and referrals for Young People both across Cherry Creek and for them to access upon their release. Engage Cherry Creek staff in community development and capacity building activities, with a focus on trauma informed care, recovery orientated care and other contemporary mental health and AOD trends. As the Primary Care Mental health AOD Clinician you will: Provide primary health-level assessment, treatment and recovery based interventions to Young People in custody at Cherry Creek who are experiencing mental health difficulties. Provide a range of harm reduction services and education, including delivery of manualised AOD programs, to groups and individual Young People in custody at Cherry Creek. Facilitate holistic care by providing appropriate support, linkages and referrals for Young People both across Cherry Creek and for them to access upon their release.
