QUALIFICATIONS:

• Bachelor’s degree in Social Work from an accredited university
• Active registration with the Ontario College of Social Workers
• At least two years of experience in health care. Experience in geriatrics and/or rehabilitation preferred.
• Bilingualism is considered an asset
• COVID Vaccination required to work in our facility (2 Doses)

• Providing support before, during and after transitions of care
