Responsibilities
  • Goal Setting: Collaborate with clients to establish personalized short- and long-term rehabilitation goals.
  • Interdisciplinary Team Coordination: Coordinate with healthcare professionals (physicians, therapists, social workers, etc.) to ensure seamless, integrated care.
  • Client-Centered Rehabilitation Services: Deliver functional supports, cognitive communication therapies, and vocational, educational, and social integration support.
  • Resource Navigation & Community Integration: Assist clients in accessing community programs and resources that foster progress and independence.
  • Education & Family Involvement: Provide training, education, and resources to clients and families to strengthen care strategies.
  • Care Review & Planning: Conduct comprehensive reviews of client files, evaluating past interventions and recommending future care plans.
  • Rehabilitation & Life Skills: Support development of daily living skills, coordinate therapies (physical, occupational, speech, cognitive, behavioral), and facilitate recreation programs.
