Description

We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.
A fixed term 12 month post for a Physiotherapist has become available in the East Dunbartonshire Community Rehab Team within the Home for Me service in East Dunbartonshire HSCP. The service provides an integrated assessment and response function to prevent avoidable hospital admissions, facilitate timely discharge, provide rehabilitation to intermediate care beds and provide a period of rehabilitation within the community.
The post holder will provide specialist multidisciplinary client assessment, case coordination, treatment and proactively support planned discharge for adults with complex needs. The post has a particular focus of working at the interface of acute and primary care to support early discharge and prevent avoidable hospital admission. The post also covers the Intermediate Care beds within the HSCP and you would be expected to carry out assessment and treatments within this area in order to facilitate discharges to own homes.
The experienced practitioner is responsible for planning, co-ordinating, delivering and evaluating the Physiotherapy service provided as part of the Rehabilitation Service and will include liaison with targeted areas within Acute to facilitate discharge and prevent avoidable admission. This may include in-reach to acute sites.
It is essential you have a Degree in Physiotherapy or equivalent with current HCPC registration. You should have a minimum of 2 years post graduate experience, and proven clinical expertise in a wide range of areas. NHS experience in both acute and community settings is desirable. A full driver’s license is required for this post.
