JOB OVERVIEW

We are seeking a Private or Family Physician in Jurisdiction C States only, to partner with us in providing referrals for Durable Medical Equipment (DME) including:

  • Glucose Monitors
  • Commodes
  • Off-the-shelf Orthotic Braces

The physician will be responsible for submitting 10–20 referrals monthly for eligible Medicaid & Medicare patients within Jurisdiction C states, only.

QUALIFICATIONS

  • Medical degree (MD or DO) from an accredited institution.
  • Valid medical license to practice in the state of employment.
  • Experience in hospital medicine or outpatient settings preferred, with exposure to various specialties such as pediatrics, obstetrics/gynecology, or geriatrics being advantageous.
  • Proficiency in using EMR/EHR systems; familiarity with Epic is a plus.

Responsibilities
  • Conduct thorough physical examinations and medical assessments of patients.
  • Diagnose and treat a variety of medical conditions across multiple specialties including but not limited to cardiology, endocrinology, gastroenterology, and psychiatry.
  • Utilize EHR systems for accurate documentation and management of patient records.
  • Perform procedures such as suturing, airway management, and catheterization as necessary.
  • Collaborate with multidisciplinary teams in critical care settings and during perioperative care.
  • Provide health coaching and education to patients regarding lifestyle changes and disease management.
  • Participate in clinical research initiatives to advance medical knowledge and improve patient outcomes.
  • Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
  • Engage in discharge planning and coordinate post-acute care for patients transitioning from hospital to home or other facilities.
