Description Essential Functions: Resident Care · Provides medication assistance for residents in accordance with state regulations and company policy including: pouring, preparing, assisting with administration, and documentation · Assists in maintaining the medication cart and storage room in a neat and orderly manner · Accepts responsibility for accurate medication counts and for security of the medication cart and medication storage room for assigned shifts · Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, eating, transferring, and mobility, as assigned. · Engages residents in meaningful conversation, socialization, and activity while providing personal care and medication assistance · Responds to all resident needs and/or requests for care promptly, and provides or obtains assistance with care as necessary · Always upholds Residents’ Rights · Supports and upholds all company policies and care philosophy · Observes residents for changes in physical, emotional, mental, or behavioral condition, unusual symptoms, medication side effects, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurse · Completes all documentation such as medication administration records, incident reports, change of condition reports, and behavior tracking, as requested, and in a timely manner · Assists in the dining room during resident mealtimes as needed · Encourages residents to participate in scheduled activities and outings, and provides residents with opportunities for self expression and enhanced self esteem through spontaneous individual or small group interaction and activity · Must be able to work full-time or part-time as agreed upon, on assigned shift, including weekend and holiday rotations · Assist residents with activities of daily living including bathing, dressing, grooming, toileting, incontinence care, transferring, ambulation according to their individualized service plans. Participate in service plan meetings for specified residents. · Participate in housekeeping/laundry tasks to provide a clean and safe environment for residents/staff to work and live in; this may include: bed making; laundry of residents’ personal items, towels and flat linens; maintaining cleanliness of surfaces and floors in individual apartments and common areas. · Assist with dining services: plating of food in dining room, serving residents, cutting food, assisting with feeding by offering encouragement, cueing, providing hand over hand guidance and/or feeding. · Participate in life enrichment programming to include gathering of residents; escorts to/from, supervision and leading of programs; and in family and social activities. · Report and record ANY changes in resident condition (physical, psychological, behavioral, social) to nurse on duty. · Document resident status per community policy. · Apply principles of infection control, including standard precautions, hand hygiene, personal protective equipment, cleaning and disinfection, respiratory hygiene, waste disposal. · Monitor resident environment to ensure all safety standards are met or exceeded. · Participate in the admission of new residents. · Promptly respond to all pendants, call lights and pull cords ensuring highest level of hospitality and customer service. Anticipate resident needs as much as possible. · Routinely make rounds of resident care areas to ensure resident wellbeing. · Provide emergency response within scope of practice · Follow through with all assignments completely, thoroughly and timely. Work collaboratively with team members to ensure success of community. Other · Attends all required staff meetings and in-service training sessions · Demonstrates a positive attitude and contributes to an atmosphere of team spirit and cooperation within the Community · Answers phones as needed, in a prompt and courteous manner · Promotes customer service and positive community relations by treating residents, families, other staff, and all visitors in a courteous, helpful and friendly manner · Assists with other duties as assigned Requirements · High school diploma or equivalent preferred · Minimum of 1-year previous experience as a caregiver for the elderly in a residential or long- term care setting · Must be able to read, write and speak English to communicate effectively with residents, families, visitors, and other staff members · Must be able to perform job duties and responsibilities with or without reasonable accommodation · Must complete Medication Technician training in-house. · Climbs, reaches, bends, and/or twists · Talks and listens frequently · Medium work – Ability to exert 20-50 pounds of force occasionally, and/or 10-20 pounds of force frequently, and/or up to 10 pounds of force constantly to lift, carry, push, pull or otherwise move objects. · Must be able to see well enough to write and read, and to visually interact with residents, families, and staff. · Must be able to speak and hear well enough to verbally interact with residents, families and staff.
