Primacare Living Solutions is currently looking to fill a role of a Resident Care Coordinator for our Long-Term Care Home in St. Catharine’s.
In this role the incumbent will provide Social Service work support to our residents and their families.

QUALIFICATIONS

• Membership in the Ontario College of Social Workers
• Experience working in a similar or same role in Long Term Care
• Knowledge of applicable regulations
  Job Type: Permanent

Schedule:

• 8 hour shift

Experience:

• Long Term Care: 2 years (preferred)

Licence/Certification:

• Ontario College of Social Workers (required)

• Providing Support and Counselling to Residents and Families
• Facilitating admissions, discharges and transfers
• Facilitating tours of the home for new residents and families
• Attending and participating in Care Conferences
• Work with all members of the interdisciplinary team
