Description Job Overview: The Licensed Vocational Nurse (LVN) is responsible for providing nursing services within the scope of practice permitted by the California Board of Vocational Nursing and Psychiatric Technicians (BVNPT). In this role, the LVN supports resident health and wellness by administering medications, training, monitoring resident conditions, documenting care, and assisting with individualized service plans. The LVN also provides supervision and guidance to care staff, ensuring compliance with California Title 22 regulations, facility policies, and best practices in assisted living. This position requires fluency in English, proficiency with computer systems (including electronic health records and medication management platforms), and the ability to work in a physically active environment that involves frequent walking, standing, bending, and lifting. The LVN must demonstrate professionalism, compassion, and strong communication skills while fostering a safe, supportive, and dignified living environment for residents. Resident Care o Perform nursing assessments and document resident status in accordance with California regulations and facility policy. o Administer medications and treatments accurately per physician orders and Title 22 guidelines. o Monitor residents for changes in condition; promptly report and document findings to the physician and Resident Services Director. o Assist with development, implementation, and review of individualized care plans. o Provide direct care as needed, maintaining a resident-centered approach. Staff Support & Oversight o Supervise, train, and support caregivers/med techs in providing quality resident care. o Ensure proper medication documentation and compliance with medication management systems. o Support staff in recognizing and reporting changes in resident condition. Compliance & Documentation o Maintain accurate, timely, and complete resident records in accordance with state law and facility standards. o Comply with all applicable regulations including California Title 22, DSS Community Care Licensing Division (CCLD) requirements, and OSHA safety standards. o Assist with audits, incident reporting, and regulatory surveys as requested. Family & Community Relations o Communicate effectively with residents, families, and outside healthcare providers. o Provide education and support to residents and families regarding health conditions, medications, and care needs. o Represent the facility’s mission and values with professionalism and compassion. Requirements Education & Experience o Current California LVN license in good standing (required). o Must be able to read, write, and speak English fluently. o Current CPR and First Aid certification (required). o Prior experience in assisted living, memory care, or long-term care preferred. o Knowledge of California Title 22 and DSS licensing regulations strongly preferred. Skills & Abilities o Proficiency with computer systems, including electronic health records (EHR), medication management systems, email, and Microsoft Office. o Strong documentation and organizational skills with attention to detail. o Ability to supervise, coach, and support unlicensed staff. o Excellent communication and interpersonal skills with residents, families, staff, and outside providers. o Ability to remain calm and make sound decisions in emergency situations. o Compassionate, resident-centered approach to care. Physical Requirements o Ability to lift, push, pull, or carry up to 50 lbs. unassisted. o Frequent standing, walking, bending, stooping, and reaching throughout the shift. o Must be able to move quickly in emergency situations. o Ability to tolerate prolonged periods of physical activity and occasional stress. Working Conditions o Work is performed in a licensed Assisted Living Facility environment. o Exposure to residents with memory loss, mobility limitations, or complex medical needs. o May be exposed to infectious waste, diseases, conditions, and unpleasant odors. o May occasionally be exposed to varying outdoor weather conditions while assisting residents. o Fast-paced, resident-centered environment requiring flexibility and teamwork.
