In this role, you’ll assess new admissions, develop individualized care plans, communicate with families and providers, and ensure residents receive the highest quality of care. Valid CPR certificate. Experience Six months experience in a long-term care environment preferred. Must have, as a minimum, one (1) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
