What You'll Do As the Resident Care Manager, you will: Develop, implement, evaluate, and update individualized, person-centered care plans. Complete comprehensive assessments for new admissions and significant changes in condition. Partner with physicians, therapists, social services, and interdisciplinary team members to coordinate exceptional resident care. Participate in daily clinical discussions and care conferences to ensure optimal outcomes. Utilize the electronic medical record (EMR-POINT CLICK CARE) to accurately document assessments, interventions, physician orders, and resident progress. Monitor residents' health status and proactively identify changes in condition. Communicate resident condition changes using the SBAR communication process. Review laboratory and diagnostic results and coordinate appropriate follow-up. Implement physician orders and ensure timely completion of treatment plans. Educate residents and families about diagnoses, treatments, medications, and care plans. Provide emotional support and guidance to residents and their loved ones. Ensure medical records are complete, accurate, and survey-ready. Foster a culture of teamwork, compassion, accountability, and outstanding customer service. Maintain reliable attendance and contribute positively to the clinical team. Additional Responsibilities Coordinate discharge planning and smooth transitions to the next level of care. Collaborate with hospitals, physicians, insurance providers, case managers, and community partners to facilitate admissions and discharges. Communicate anticipated discharge needs with the interdisciplinary team. Promote regulatory compliance by following company policies, professional standards, and all applicable federal and state regulations. Perform additional duties as assigned to support resident care and facility operations. Requirements Education Degree in Nursing from an accredited nursing school. RN required/preferred. Licenses/Certification Current RN license. Valid CPR certificate. Experience Six months experience in a long-term care environment preferred. Minimum, one (1) year of experience as a supervisor in a hospital, nursing care facility, or other related health care facility preferred.
