Description BASIC PURPOSE: The Resident Liaison supports multiple departments with admissions, assessments, care plans and other duties. This position assists in mediating issues that arise among residents, families and transferring institutions. MAJOR WORK ACTIVITIES: 1. Assists in gathering social history and psychosocial assessment that includes the resident’s problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan. 2. Orients residents and families to the facility, services offered, resident’s rights and assist residents and their families during the transfer phase. 3. Maintains and keeps up to date FL2’s, assessments, care plans, and other forms according to state regulations and company policy. 4. Assists in creating individualized resident care plan to include involving families in care planning, encouraging their attendance at care conferences and scheduling care conferences and identifies and interprets psychosocial needs of residents for inclusion in the care plan, addressing in particular issues of schedule, treatment, and environment that express and reinforce individuality and identity. 5. Mediates issues that arise among residents, families, and staff. 6. Ameliorates emotional distress of residents and families. 7. Assists residents and families with financial questions and makes appropriate referrals. 8. Act as liaison between transferring institutions and the facility by maintaining good rapport. 9. Plans and directs discharges/transfers to ensure appropriateness and continuity of care for transfers within and discharges from the facility in collaboration with residents, families, multidisciplinary staff and referral agencies, which identify resident needs and preferences. 10. Supports the Sales & Marketing by coordinating move ins including all paperwork and assisting with tours. Requirements MINIMUM QUALIFICATIONS: Education: High School Diploma/ GED. Bachelor's degree in human services field preferred. Licensure/ Certification: N/A Experience: At least 18 months of previous social work experience in geriatric setting. An equivalent combination of education and experience may be considered.
