RESIDENT SERVICES CASE MANAGER Hultquist Place | Jamestown, NY Status: Full-Time Reports To: Director of Admissions & Case Management Pay Range: $24.00 - $32.25/hour (based on experience and qualifications) Make a Difference Every Day! Hultquist Place, a 96-bed Adult Home providing Assisted Living Program (ALP) and Licensed Home Care Services Agency (LHCSA) services, is seeking a compassionate, organized, and resident-focused Resident Services Case Manager. This newly created position serves as the primary non-clinical advocate and service coordinator for residents, helping individuals and families navigate services, benefits, community resources, admissions, transitions, and quality-of-life needs while promoting independence, dignity, and well-being. KEY RESPONSIBILITIES RESIDENT SUPPORT & ADVOCACY * Assist residents with adjusting to community living. * Educate residents and families about resident rights and available services. * Advocate for resident rights, dignity, and choice. * Support residents experiencing emotional, social, or psychosocial challenges. * Assist with concerns, grievances, and conflict resolution. * Provide support and communication for families as appropriate. CASE MANAGEMENT SERVICES * Complete psychosocial assessments upon admission and as required. * Maintain ongoing case management documentation and progress notes. * Monitor resident social, emotional, and behavioral well-being. * Identify service needs and coordinate appropriate interventions. * Participate in interdisciplinary meetings and care conferences. * Communicate resident concerns to nursing, admissions, administration, and other departments. BENEFITS & RESOURCE COORDINATION * Assist residents and families with Medicaid applications and recertifications, SSI, Medicare-related concerns, transportation resources, and other benefit programs. * Coordinate referrals to healthcare providers, behavioral health providers, and community agencies. * Help residents access available public and private resources. * Assist with appointment coordination and follow-up services. ADMISSIONS & TRANSITION SUPPORT * Support admission preparation and resident onboarding. * Obtain necessary documentation from families, referral sources, and providers. * Assist with move-in coordination and resident orientation. * Support residents and families during transitions into the community. * Participate in discharge planning and transitions to other levels of care when appropriate. RESIDENT & FAMILY ENGAGEMENT * Participate in Resident Council and Family Council activities. * Encourage resident participation in activities and community life. * Support quality-of-life initiatives and resident satisfaction efforts. DOCUMENTATION & COMPLIANCE * Maintain accurate and timely case management records. * Complete required assessments, reports, and follow-up documentation. * Support regulatory compliance, quality improvement activities, audits, and survey readiness. QUALIFICATIONS MINIMUM QUALIFICATIONS One of the following: * Associate Degree in Human Services, Psychology, Social Work, Sociology, Gerontology, Healthcare Administration, Rehabilitation Services, Occupational Therapy Assistant, Recreational Therapy, or a related field; OR * High School Diploma/GED with at least three years of experience in resident services, case management, social services, healthcare coordination, assisted living, adult care services, or related work supporting older adults. PREFERRED QUALIFICATIONS * Bachelor's Degree in Human Services, Social Work, Psychology, Gerontology, Healthcare Administration, or a related field. * Experience in Adult Homes, Assisted Living Programs (ALP), LHCSA, Skilled Nursing Facilities, Independent Living, Adult Day Services, or community-based healthcare settings. * Experience with resident advocacy, benefits coordination, discharge planning, and community resource navigation. KNOWLEDGE, SKILLS & ABILITIES * Excellent interpersonal and communication skills. * Knowledge of community resources and public benefit programs. * Strong organizational and documentation abilities. * Ability to maintain confidentiality and manage multiple priorities. * Strong problem-solving and conflict-resolution skills. * Commitment to person-centered care and resident advocacy.
