What You’ll Do Champion residents’ best interests and protect vulnerable individuals Conduct psychosocial assessments and develop person-centered care plans Collaborate with nurses, physicians, and interdisciplinary teams Guide families through resources, referrals, and community services Lead discharge planning to ensure smooth transitions home or to other care settings Address behaviors proactively to enhance safety and reduce unnecessary medication use Coordinate medication reviews to support gradual dose reduction Serve as a liaison between residents, families, and community agencies Maintain accurate, timely clinical documentation What You Bring Strong knowledge of social work practices, especially within geriatric populations Excellent communication and organizational skills A collaborative mindset and team-oriented approach Ability to maintain strict confidentiality Experience consulting with LCSW, LSW, or MSW professionals What We’re Looking For A compassionate professional who: Advocates fiercely for resident dignity and safety Understands the social and emotional factors affecting health Thrives in a collaborative, fast-paced healthcare environment Is committed to ethical, person-centered care Important Requirement All employees must successfully complete and clear state-specific background check requirements prior to resident contact. Licenses/Certification Must have a valid driver’s license. Experience Six months experience in a long-term care environment preferred.
