JOB SUMMARY AND QUALIFICATIONS: The Residential & Community Services Coordinator is responsible for overseeing the daily operations of a residential home while supporting individuals with intellectual and developmental disabilities (IDD) in achieving meaningful community inclusion, personal growth, and independence. This position ensures high-quality, person-centered services that promote dignity, choice, self-determination, and community engagement. The Residential & Community Services Coordinator (RCSC) provides person-centered, community-based support services to individuals with intellectual and developmental disabilities (IDD) receiving services through Kentucky's Supports for Community Living (SCL) Waiver. The RCSC supports individuals in building skills, relationships, and natural supports that promote meaningful participation in their communities and increased independence. This position emphasizes community inclusion, person-centered planning, skill development, and reduced reliance on paid supports, in alignment with Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (DBHDID) values and SCL Waiver requirements. The RCSC serves as a mentor, advocate, leader, and community connector, ensuring compliance with state regulations, agency policies, and best practices while fostering a safe, healthy, and supportive home environment. This position works collaboratively with individuals served, families, guardians, community partners, and interdisciplinary team members to support each person's goals, preferences, and desired life outcomes. QUALIFICATIONS: Minimum Requirements * Must meet SCL Waiver personnel and training requirements for Community Access Services and Residential Services. * Ability to communicate effectively with individuals, families, and team members. * Ability to read, understand, and implement written and verbal instructions. * Ability to complete required documentation accurately and independently. Preferred Qualifications * Prior experience providing Community Access or waiver-funded services in Kentucky. * Completion (or willingness to complete) DBHDID-approved SCL training within required timeframes. * Knowledge of person-centered practices, community inclusion, and natural supports. Additional Requirements * Valid driver’s license, reliable transportation, and ability to travel locally within Northern Kentucky. * Ability to work flexible hours, including evenings and weekends as needed. RESPONSIBILITIES: Home Management * Oversee the daily operation of assigned Hopeful Home(s), ensuring a safe, healthy, and supportive living environment for all individuals served. * Ensure compliance with Kentucky Supports for Community Living (SCL) Waiver regulations, HCBS Settings Rule requirements, agency policies, and all applicable state and federal standards. * Monitor the health, safety, and welfare of individuals receiving services, including coordination of medical appointments, medication oversight, and healthcare follow-up as needed. * Conduct regular home inspections to ensure cleanliness, safety, accessibility, and maintenance standards are maintained. * Assist with household budgeting, grocery shopping, meal planning, and management of household supplies. * Ensure individuals' rights, privacy, dignity, and personal choices are respected and promoted at all times. * Maintain a home environment that encourages independence, community participation, and person-centered living. * Respond appropriately to emergencies, incidents, and crisis situations and participate in on-call responsibilities as assigned. Community Access, Inclusion & Skill Development * Support individuals in participating in meaningful community activities, including educational, recreational, civic, volunteer, employment-related, faith-based, and social opportunities. * Assist individuals in identifying and pursuing personal interests, goals, and valued social roles within their communities. * Provide instruction, coaching, and support to develop skills related to: * Independent functioning and daily living * Socialization and relationship-building * Self-advocacy and self-determination * Personal responsibility and decision-making * Community membership and active participation * Transportation and navigation of community resources * Promote the development of natural supports, friendships, and social networks to increase independence and reduce reliance on paid services. * Build and maintain positive relationships with community organizations, employers, volunteer agencies, educational institutions, and recreational providers to expand opportunities for individuals served. * Encourage and support individuals in exercising informed choice and taking increasing responsibility for their personal goals and community involvement. Person-Centered Planning & Service Delivery * Deliver services consistent with each individual's Person-Centered Service Plan (PCSP), Individual Service Plan (ISP), and identified outcomes. * Participate as a member of the person-centered planning team and contribute information regarding progress, barriers, strengths, and opportunities for growth. * Support individuals in achieving goals that promote independence, self-sufficiency, community integration, and quality of life. * Monitor progress toward identified outcomes and adjust supports as needed in collaboration with the interdisciplinary team. * Ensure services are delivered in accordance with person-centered principles and the individual's preferences, strengths, and desired life outcomes. Staff Leadership & Team Development * Supervise, mentor, and support Direct Support Professionals assigned to the Hopeful Home. * Provide ongoing coaching, training, and performance feedback to staff. * Assist with staff scheduling to ensure adequate coverage and continuity of care. * Promote a culture of accountability, teamwork, professionalism, and person-centered service delivery. * Model positive leadership and support staff in implementing best practices for residential and community-based services. Documentation, Compliance & Quality Assurance * Complete accurate, timely, and thorough documentation in accordance with Medicaid, DBHDID, SCL Waiver, and agency standards. * Maintain service notes, progress reports, incident reports, health records, and other required documentation. * Adhere to all Kentucky Administrative Regulations (KAR) governing SCL Waiver services, including rights protection, abuse, neglect, and exploitation reporting requirements. * Follow agency policies regarding HIPAA, confidentiality, incident management, risk management, and recordkeeping. * Participate in audits, quality assurance reviews, inspections, and corrective action activities as required. * Ensure documentation accurately reflects services provided, individual progress, and compliance with regulatory requirements Health, Safety & Risk Management * Promote and maintain a safe environment both within the residential setting and throughout community activities. * Identify and address health, safety, behavioral, and environmental risks through proactive planning and intervention. * Implement safety protocols and emergency procedures as required. * Ensure incidents, injuries, medication errors, and unusual occurrences are reported and documented according to agency and regulatory standards. * Protect the rights, welfare, and well-being of individuals receiving services at all times. Work Environment & Service Delivery Expectations * Services may be provided in residential settings, community locations, workplaces, volunteer sites, educational environments, recreational venues, and other integrated community settings. * Work schedules may include daytime, evening, weekend, holiday, and on-call hours to meet the needs of individuals served. * Travel throughout Northern Kentucky may be required to support community access, appointments, activities, and agency operations. * Maintain a flexible, person-centered approach to service delivery that promotes independence, inclusion, and meaningful community engagement. Responsibilities
