Description Department: Programs and Services FLSA Exemption Status: Non-exempt, full-time Location: Denver, CO Supervisor: Case Management Program Manager Position Overview: The general purpose of the Residential Housing Case Manager role is to provide 1:1 housing navigation support for members residing at TGP’s Elati Village micro-community, a non-congregate shelter specifically serving women and gender expansive individuals exiting street homelessness. The Residential Housing Case Manager will play an essential role in delivering housing navigation support in collaboration with HOST’s Housing Central Command team. The Residential Housing Case Manager will primarily help secure stable housing for Elati Village micro-community members but will be required to provide navigation support at other sites as needed. This position will require a high level of coordination across multiple teams and programs as well as the ability to travel using a personal vehicle throughout the city. Core Accountabilities Demonstrates respect, cultural humility, and knowledge of trauma informed care in interactions with TGP members, staff, and collaborative partners. Works directly with program participants, on site at TGP and in the community, to provide trauma-informed and member-directed housing navigation support using culturally responsive and evidenced-based practices. Works directly with program participants in the community doing in-home visits, providing trauma-informed and member-directed housing stabilization support using culturally responsive and evidenced-based practices. Evaluates progress of program participants and adapts case management plan and/or interventions as needed to increase engagement. Provides crisis intervention as needed. Collaborates and communicates with the Elati Village team, TGP programs, and external partners to ensure high quality care. Is knowledgeable about and stays up to date on housing resources and other community resources that aid in recovery from homelessness, substance misuse, mental health issues, and trauma, with the goal of achieving greater stability and housing. Receives training for TGP’s member database, ETO, and the Homeless Management Information System (HMIS) and maintains monthly compliance with ETO and HMIS data tracking, assessments, etc. Enroll clients into HMIS and ETO and follow data standards including case notes and client documentation. Provide weekly check-ins by phone, email, or in person with members on the caseload to build rapport and increase supportive services. Stay up to date with HUD’s Fair Market Rent limits and find housing units within these limits in the Denver Metro and surrounding areas (Low Income Housing Tax Credit Properties, Affordable Housing, Subsidized Housing, etc.) Work in multiple locations as assigned by Housing Central Command to meet with clients in the site where they are based Attend two daily meetings led by HCC administration team. Conduct Pre-Housing navigation over a 30-day period with individuals assigned to the Navigator through Housing Central Command. Work with clients to complete housing applications at units identified by Unit Team. Transport clients as needed for pre-move in and move in tasks (vital doc acquisition, unit tours, Landlord meetings, physically moving client belongings, etc.). Work with Housing Central Command Administration Team and clients to schedule move in and furniture deliveries. Support intake and housing stabilization tasks as needed. Requirements Requirements: Must have skills in crisis management, conflict management, and de-escalation. Previous housing navigation experience a plus Lived experience of homelessness, housing instability, or poverty a plus Strong understanding of trauma-informed care principles and skills Ability to listen compassionately and suspend judgment in order to accommodate the needs of individuals and include them in a highly diverse setting Ability to multi-task in a unique, fast-paced environment Ability to work independently or with a team Critical thinking and problem-solving skills Current knowledge of, or desire to learn, how to operate a computer and other office machinery, such as a copy machine or printer, and office software, such as Microsoft Outlook, Word, Excel, and other programs.
