ROLE DESCRIPTION

As a Residential in Reach Nurse Consultant, you’ll play a key role in delivering timely, evidence-based care to older persons across public and private aged care facilities. Working closely with the RIR Lead, At Home Program Nurses, General Practitioners (GPs), and aged care teams, you’ll help prevent unnecessary hospital admissions by providing early intervention, acute care, and chronic disease management in the resident’s home or facility.

KEY RESPONSIBILITIES

• Provide direct clinical care, triage, and intake assessments for residents referred to the RIR program.
• Collaborate with RACF staff, GPs, and multidisciplinary teams to develop and implement individualised care plans.
• Support residents and carers to access appropriate at-home care options including Hospital in the Home (HITH).
• Monitor and respond to acute deterioration, ensuring timely escalation and transition planning.
• Advocate for optimal health outcomes through education, health promotion, and coordinated care.
