Coordinate Wellness: Assist with home-care arrangements, and partner with our rehab team. Advocate & Educate: Assist residents in understanding advance directives, insurance transitions, and community resource options. Bridge the Gap: Act as the vital link between hospital staff, families, and our internal clinical team during hospitalizations or health changes. Responsibilities

The Care Navigator guides residents and families through transitions between different levels of care and coordinates wellness and home-care arrangements. They also act as a liaison between families, hospital staff, and the internal clinical team to ensure seamless health transitions.
