We are a vibrant, non-profit life plan community with approximately 700 residents, and we are seeking a dedicated Resource Navigator to join our team. As a Resource Navigator, you’ll be at the forefront of our mission to promote health and wellness through our eight dimensions of wellness. Your Role: As a Resource Navigator, you will serve as a resident advocate, offering support and guidance across various aspects of life planning, financial support and educational opportunities. Your work will primarily focus on Independent Living but may extend across our entire campus. Key Responsibilities: Life Planning: Assist residents in understanding and completing essential life planning documents, including living wills, health care POAs, financial POAs, and long-term care policies. Financial Supports: Guide residents through available financial support programs like AZ Long-Term Care (ALTCS), Resident Assistance Fund (RAF), and Veterans' Benefits (VA). Community Resources: Provide trusted referrals to community partners and services, including legal, financial, and insurance assistance. Educational Events: Organize and facilitate seminars and classes on health, wellness, and aging-related topics. Expand offerings based on your expertise and interests. Transition Care Support: assisting residents as they transition through different levels of care (e.g., independent living, assisted living, skilled nursing, or other internal support services.) Requirements What We’re Looking For: A BSW in social work, or bachelor's/masters degree in a related field from an accredited college or university. At least 2 years of community support experience working with older adults with one year experience in a Life Plan Community Strong problem-solving and resourcefulness in identifying appropriate support services. A genuine passion for working with older adults in a setting that honors autonomy and choice. An understanding of the complexities of aging and a commitment to lifelong learning and growth.
