We have a full-time vacancy in our Hastings Community Hospital Team.
As a Resource Officer in Adult Social Care, you will play a key role in undertaking assessments and reviews of adults and carers who have been referred to your team. Specifically, you will undertake Care Act assessment of needs and care and support plan reviews of adults who are ready for discharge from two of our Community Hospitals in East Sussex. You will be expected to work in a multidisciplinary team to support a person’s discharge and contribute to discussions that promote the person’s independence and best interest principles.
Once you have undertaken training, you will be expected to hold and manage safeguarding enquiries for Adults who are subject to abuse and/or harm and fulfil our duty under s42.2 of the Care Act 2014 by working with partner agencies to assess, and minimise safeguarding risk to residents of East Sussex.
We have excellent relationships with local partners and hospital trusts, and you will have the opportunity to work alongside other professionals to ensure good outcomes for the individual’s you are working with. Where necessary, we work with our local Integrated Commissioning Boards (ICB’s) to ensure people we serve get access to continuing care funding.
We are looking for candidates with experience of working with people who access social care support.
