We’re looking for a skilled and flexible RN Admission & Discharge Nurse to coordinate resident admissions, transfers, and discharges—ensuring each transition of care is safe, smooth, and well-coordinated. What You’ll Be Doing Coordinating and completing resident admissions, discharges, and transfers Completing thorough admission assessments, identifying and staging wounds, and ensuring accurate documentation and timely communication of findings. Receiving hospital hand-off reports and ensuring continuity of care Entering and verifying physician orders in the EHR so MARs and TARs are spot-on Sending demographics and medication orders to pharmacy for timely fills Collaborating with nursing, therapy, social services, providers, and families Being the calm, organized presence that keeps transitions stress-free When admissions volume is low, provide support to the nursing leadership team with survey preparedness activities, audits, and regulatory compliance efforts.
