What You Should Know About the RN Admission Care Liaison Role: * The Admission Care Liaison is a DRIVING position supporting Ohio's Hospice South Region which cover the greater Miami Valley and surrounding areas * Schedule hours are day/evening shift 10am-10:30pm with every3rd weekend rotation and 1-2 holidays per year * We provide superior care and superior services to patients at their end of life journey.  Only those who have a heart for hospice will succeed   The RN Admission Care Liaison Essential Duties Are: Ohio’s Hospice patients will receive quality nursing care and coordination of hospice services, when the RN/LPN Admissions Care Liaison admits the patient into the hospice program efficiently and provides the patient and family with the appropriate resources needed at the end of life. The RN/LPN Admissions Care Liaison will maintain a conversion rate of 75% and skillful use of the electronic medical record by accurate documentation at the point of care.   RN Admissions Care Liaison Qualifications:   RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.  Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred. * CPR Certified   * Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)   * Computer skills sufficient to properly document services and care.   * Ability to drive during daytime, nighttime or inclement weather.    * Valid Driver's license with Safe Driving Record. * State minimum automobile insurance coverage. * Must be able to mass a background check * Must be able to pass a 10 panel drug screen  Benefits & Perks: your health and happiness mattes!
