Qualifications:  * Graduate of an accredited school of nursing. * Associates degree in nursing. * Registered as RN with New Hampshire’s Board of Nursing. * A minimum of one (1) year recent acute care experience. * Ability to exercise initiative and independent judgment. * Excellent verbal and written communication skills and competent in problem solving abilities.  Responsibilities * Admission to HHHC's homecare division. * Completes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). * Utilizes the nursing process in planning, providing, and evaluating skilled needs. * Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring. * Develops a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.  Preferred Qualifications * Bachelor’s degree in nursing. * Community Health/Education experience. * Certifications, which may include: IV; Chemotherapy; Ostomy. * Familiarity with OASIS and what drives reimbursement. * Supervisory experience over home health aides.  Other Requirements: * CPR Certification. * Satisfactory criminal record. * Satisfactory motor vehicle record.
