Conduct assessments, create individualized care plans, and support patients through their visit or transition to post‑acute care. Administer medications, perform treatments, and ensure patient and family education throughout the care experience. Prioritize clinical needs while maintaining a calm, professional presence. Collaborate with the interdisciplinary team to ensure seamless communication and optimal patient outcomes.
